Pseudo-secondary hyperparathyroidism due to vitamin D deficiency and coexisting familial hypocalciuric hypercalcaemia

Mitchell Anna Louise , Bliss Richard , Pearce Simon H S

A 54-year-old lady presented with malaise, weakness, and constipation. She was found to be mildly hypercalcaemia (adjusted calcium 2.68–2.76 mmol/l; reference range 2.12–2.60 mmol/l) and was referred to local endocrinology services. Her PTH was 79 ng/l (range 10–60) and a diagnosis of primary hyperparathyroidism was made. Imaging to localise a parathyroid source was negative. She was referred to the endocrine surgeons at the Royal Victoria Infirmary, Newcastle, ...

Impact of COVID-19 pandemic on waiting time from referral to definitive surgery in primary hyperparathyroidism: a large tertiary centre experience

Rashid Razan Ali , Ramsingh Jason , Bliss Richard , Truran Peter , Mamoojee Yaasir

Background: The COVID-19 pandemic has resulted in widespread disruption to delivery of emergency and elective care in the last 2 years. Nevertheless, healthcare systems have quickly readapted to accelerate use of novel pathways for delivering clinical services. We reviewed the impact of COVID-19 pandemic on our time from community referral to definitive surgery in patients with Primary Hyperparathyroidism (PHPT).Methods: We retrospectively reviewed the w...

What is the diagnostic serum ACTH threshold in adrenal Cushing?

Naveen Aishwarya , Guma Muna , Mamoojee Yaasir , Bliss Richard , Ramsingh Jason , Truran Peter

Background: Diagnostic work-up for Cushing Syndrome (CS) can be challenging and is based on clinical and biochemical assessments. Once steroid excess is biochemically confirmed, the diagnostic serum ACTH threshold for determining ACTH-independent Cushing is debatable. A threshold of less than 15 ng/l has been adopted in our regional guidelines.Aims: To investigate serum ACTH levels in patients with proven adrenal Cushing and to assess the clinical utilit...

What is the prevalence of severe post-operative hypocalcaemia in patients who have undergone parathyroid surgery or a total thyroidectomy at the RVI, Newcastle? Does vitamin D play a role?

Pawlak Anna , Quinton Richard , Truran Peter , Lennard Thomas , Bliss Richard , Heed Andrew

Background: Hypocalcaemia is a common electrolyte disorder and it can occur following parathyroidectomy or thyroidectomy. It has been suggested that some post-operative hypocalcaemic crises may reflect severe vitamin D deficiency and vitamin D supplementation may have a protective effect. Objectives: To establish the prevalence of severe post-operative hypocalcaemia (as defined by the requirement for i.v. calcium gluconate) following parathyroid surgery a...
